The Spell tab on the Utility Dialog Bar is used to add words or replacement pairs to your primary user dictionary. Normally if you want to add one flagged word to your dictionary as you are working, it is easiest to use the right click menu (right-click on a flagged word and select Correct Word) or Ctrl+? (with the cursor on a questionable word, press Ctrl+?). Update Delay determines the timing of how long you have to be on a misspelled word before it updates the list. If you are in full I-want-to-correct-all-misspellings-now mode, you might want that to be virtually instantaneous. But if you are generally just writing, and want simplified spelling assist, you might not want it to do that as you simply move the cursor around in the text, but instead wait until you pause.
